Trial Period

Duration: 30 days.

Allotment: Unlimited patients.

Cost: $1.00, charged on Day 1 and non-refundable

Day 31: the trial ends and the First Paid Month begins. See "First Paid Month" below.

The Trial Period is separate from, and precedes, the First Paid Month. A practice that converts from a trial receives both, in that order. A practice that signs up directly begins at the First Paid Month.

First Paid Month (Unlimited Patients Policy)

Price: $499.00 per month, charged at the start of the first monthly billing cycle. This is a monthly subscription charge. It is not a setup, activation, implementation, onboarding, or other one-time fee.

Allotment: Unlimited patients. No per-patient and no overage charges accrue during the First Paid Month.

Duration: one monthly billing cycle. The First Paid Month ends immediately before the second monthly billing cycle begins.

Refund: to receive a full refund of the $499.00 Subscription Fee paid for the First Paid Month, the practice must submit a written refund request to support@repeatmd.com before the end of the First Paid Month. Upon receipt of a timely refund request, the practice will be entitled to a full refund of the $499.00 Subscription Fee. The refund will be issued to the original payment method. One refund per practice.

Tier assignment: before the end of the First Paid Month, Ageless will send the practice, by email and in the platform, its patient volume for the month, the tier being assigned, that tier's included volume and monthly price, the date and amount of the first charge at that price, and how to select a different tier or cancel. A practice that does not respond is automatically enrolled in the assigned tier at the price stated in that notice, effective the day after the First Paid Month ends.

Assigned tiers are monthly. A practice may elect an annual tier instead at any time.

Eligibility: new practices only, one First Paid Month per practice. Practices under common ownership, common management, or a shared brand count as one practice.

Subscription Tiers

The First Paid Month is billed at $499.00 per month with unlimited patients. From the second monthly billing cycle, the assigned tier's price and allotment apply.

Overage Fees

First overage month: free. Subsequent months: $10 per patient above tier cap. Customers may disable overages or upgrade tiers to avoid charges. No overage fees accrue during the Trial Period or the First Paid Month.

Patient Bank (Rollover Credits)

Monthly plans: 1x rollover cap, 60-day expiration. Annual plans: 2x rollover cap, 90-day expiration.

Begins accruing from the second monthly billing cycle. No patient capacity from the Trial Period or the First Paid Month is eligible for rollover.

Subscription Pause

$99/month, replacing the standard subscription fee. Maximum duration 3 consecutive months per year. Dashboard access is read-only. A subscription may not be paused during the Trial Period or the First Paid Month.

Founders Club Add-On

One-time $500 fee. 12-month term. Available during the launch window of March 16 to April 16, 2026. Non-refundable, no auto-renewal.

Reactivation

Canceled during trial: no second $1 trial. Must choose a paid plan at full price to reactivate. A practice that has received a First Paid Month, or a refund of one, is not eligible for another.

General Terms

Existing customers receive 30 days' notice before any price increase takes effect.

This Fee Schedule is incorporated into the Master Subscription and Services Agreement by reference. Capitalized terms not defined here have the meanings given in that agreement.
